Cancellation Policy and Non-Attendance Fees:

Where your doctor has referred you to this service under a care plan and associated team care arrangements, or a private patient either referred by doctor or self-referred, a cancellation or rescheduling of appointment is required 48 hours prior to scheduled appointment.  

Please be advised that to ensure that bulk billing under a care plan and associated team care arrangements remain sustainable for accessible care for all people living with diabetes can continue, a charge for late cancellations or non-attendance fees will apply.  This fee cannot be charged using the care plan and will be billed privately.   

This fee will be charged in the event of a non-attendance of appointment or in the case when less than 24 hours’ notice is given.

Terms and Conditions Acknowledgement sign off form to auto upload would be helpful in this section – possibly linked to appointment page to be complete prior to appointment being able to be booked.  

In the event of exceptional circumstances, please notify staff of issue and patient will be advised if the cancellation/non-attendance fee will stand. 

Any fees incurred are required to be paid before the next consultation is booked.

The fees incurred are $60.00 for all appointment types.
