What should I bring to my first appointment?
Medicare card, &
Private Health Details if applicable, and
Your referral if using a care plan not emailed from GP, if not sure about this, we will let you know when booking

In short no….. However, there are financial benefits under a General Practitioner’s (GP) Care Plan with associated Team Care Arrangements.  

Medicare pays rebates for Allied Health specialist services, such as Diabetes Education from credentialled diabetes educators (CDE’s), only if you have been referred by a GP.

People are welcome to self-refer to this service also. 

Your credentialled diabetes educator will review your medical history to ensure that all education incorporates any other chronic disease conditions, to ensure holistic healthcare delivery.

A physical examination will also be carried out where required and most recent pathology and current medications will be reviewed.  Further tests may be requested from your GP. Communication between your CDE and GP will occur to ensure collaborative health care continues. 

Often people may have questions and fears related to being diagnosed with any type of diabetes.

Don’t be afraid to ask anything! Write a list if that helps.

This will also help you make the most of your time with the educator, whilst ensuring piece of mind, with all your questions answered.

For follow-up appointments telehealth and video consultations via Google Meet and Teams consultations to accommodate work hours and busy lifestyles in general.  Many regional patients in Queensland also appreciate the convenience.

Telehealth consultations are conducted via a secure video link emailed to you ahead of time.

For initial consultations we would love to see you face to face to set you up with equipment and resources. However, this is not always practical, but with planning, happy to accommodate as needed. 

If you have been living with diabetes for some time, yes, a telehealth or video consultation is fine.
