What should I bring to my first appointment?

Your referral if using a Chronic Condition Management Plan or DVA Treatment Cycle of Care not emailed from GP, if not sure about this, we will let you know when booking

In short no….. However, there are financial benefits under a General Practitioner’s (GP) Chronic Condition Management Plan or with a DVA Treatment Cycle of Care.

Medicare pays rebates for Allied Health specialist services, such as Diabetes Education from credentialled diabetes educators (CDE’s), only if you have been referred by a GP.

Private patients and NDIS recipients who are self-managed or plan manged are always welcome to self-refer.

Your credentialled diabetes educator will review your medical history to ensure that all education incorporates any other chronic disease conditions, to ensure holistic healthcare delivery.

A physical examination will also be carried out where required and most recent pathology and current medications will be reviewed.  Further tests may be requested from your GP. Communication between your CDE and GP will occur to ensure collaborative health care continues. 

Often people may have questions and fears related to being diagnosed with any type of diabetes.

Don’t be afraid to ask anything! Write a list if that helps.

This will also help you make the most of your time with the educator, whilst ensuring piece of mind, with all your questions answered.

Telehealth consultations can be hosted for people who are newly diagnosed with diabetes and for those who have been self-managing living with diabetes for quite some time. Your CDE will arrange resources to be emailed and posted to you direct to ensure you have a complete service.   

Our main practice location is in Bracken Ridge (North Brisbane) with home visits available for people with mobility issues.
